In this video Dr. Roger Seheult of MedCram continues to discuss the asthma case from the video of part 1. The patient has continued to progress in her difficulty breathing and requires being placed on a ventilator. This video is to supplement part of MedCram’s previous ventilator courses

Ventilator management pearl in asthma

Dr. Seheult discusses ventilator management in this patient.  He describes the inspiratory flow rate and how this can be adjusted to aid in ventilation. He relays that the most important thing to do in a patient with hyperinflation while on the ventilator is to reduce the respiratory rate. He does concede that sometimes management of an asthma patient on a ventilator may be extremely difficult and it is important to be able to recognize that additional resources may be needed such as ECMO (extracorporeal membrane oxygenation in a heart-lung machine). If a patient is not able to be ventilated, it might require switching over to manually bagging the patient by hand to ventilate them and transferring for higher level of care at a facility where ECMO is available.
